As a convenience to our families, we are excited to partner with Healing Hands once again to offer on-campus sports physicals for students who need one before participating in after-school athletics.

Details:

• Date: Wednesday, August 19, 2026
• Time: 9:30 AM – 2:30 PM
• Cost: $25 per student
• Payment Options: Cash or Credit Card accepted

What’s Needed:

• Parents must complete and sign all AIA forms and the Healing Hands Consent form
• Students cannot be seen without both sets of forms.

Additional Notes:

• Any significant findings will be communicated directly to parents by phone.
• Students will receive their completed forms the same day (or we can hold them in the office if preferred).
• Physicals are good for one school year.

This is a great opportunity to take care of sports physicals conveniently here on campus. Please complete the attached paperwork and return it to the school office by Monday, August 10th.

If your child plans to participate in after-school sports at HCCA, a current sports physical is required before they may participate. If your child has already completed or is scheduled to complete a sports physical with their personal physician, please email a copy to athletics@heartcrychristianacademy.com no later than Wednesday, August 19th.
